Social Prescribing

Social prescribing provides a 1:1 personalised support service with a Social Prescribing Link Worker, Care Coordinator or Health & Wellbeing Coach - Please talk to reception about the service.

The Team includes:

Coffee Morning

Care with a Cuppa - Our Health & Wellbeing Team are on hand to offer practical help and support with issues such as housing, debt, benefits, social activities, employment, chronic health conditions, mental health and weight management - Care with a Cuppa is at Roseby Road Community Centre, Peterlee, SR8 4RJ. Starts  Friday 21st July 2023 9.30 till 11.30 and fortnightly thereafter. Everybody welcome. Please contact james.buchanan@nhs.net for more information
